Welcome to our all-new forms resource center!

Greater St. Stephen is a GREATER Church, doing GREATER Works to help build a GREATER Community! And none of that would be possible without dedicated and obedient leaders like YOU! Below is the forms and timeline checklist to help keep you on track as you endeavor to do GREATER Works!

Forms and Timeline Checklist

a. Event Questionnaire (Needed for every event)
b. Event Budget Form (Needed for every event)
c. Graphics/Media Request Form
d. Auxiliary Expenditure Request (If the ministry is covering the expense)
e. Check Request Form (If the church is covering the expense)
f. Ministry Facility Usage Form (For all meetings, rehearsals, and set-ups)
g. Table Request Form (if tables are needed)
h. Van Request Form (If the van or bus is needed)
i. Gas Card Request Form (If traveling with the van or bus)
j. Culinary Form (If Kitchen is needed or reception is to be set-up)
k. Announcement Request Form (To promote event)

Event Approval Timeline

2. 90 Days Out
a. Forms all completed and signed by your (Directors and Overseer)
b. Meet with Pastor of Ministries to discuss vision, and to go over the Event Questionnaire & Budget Form.
c. Meet with the Pastor of Ministries for approval of date, time, and final planning of event.
d. Have copies of all estimates, possible contracts, and invoices.

3. 60 Days Out
a. Schedule meeting with Graphics, Media, Sound, and Music Ministries.
b. All public relations forms reviewed and with a date of completion assigned by Graphics, Media, Sound, and Music Ministries.
c. Flyers, banners, slides, programs, door hangers, etc.

4. 30 Days Out
a. All Advertisement will go forth on Announcements, E blast, Social Media, GSS News, Screens, etc.
b. All Check Request with invoices, receipts, or contracts all due in.
c. All evangelism with the ministry, committee, & evangelism team will go forth.
d. 2 weeks before event a final meeting with the Pastor of Ministries to make sure everything is ready with your event

OUR POLICY:

SCREEN ANNOUNCEMENTS
Design requests for screen announcements must be submitted no later than Tuesday at 5pm.

BULLETIN ANNOUNCEMENTS/ADS
Design requests for the monthly bulletin must be submitted no later than the Tuesday before the 1st Sunday of month.

ALL OTHERS
All other request must be completed and submitted to the Pastor of Ministries at least 30 days prior to your desired date of
completion.
